Health Forms and Action Plans

Health and emergency action plans on file at school are essential for ensuring your child stays safe and supported throughout the school day. Parent/guardian should notify the district nurse of any health conditions, concerns and/or special health-related needs.

Below are the health plans available for use. If you already have a health or action plan from your health care provider, such an an Asthma or Food Allergy Action Plan, feel free to provide that to the school instead. Health forms, action plans, and all related paperwork must be updated at the start of each school year.
