If your student needs medication at school—whether daily, as-needed, or for emergencies—we are here to help! Please review the medication information below and submit the required forms/paperwork to the district nurse.

Medication Information

Prescription Medications

1. A parent/guardian permission to administer medication consent form.           

2. A medical order from your child's health care provider.  

3. Medication must come in it's original, non-expired prescription labeled container that includes the name and phone number of pharmacy, child's name, name of medication and dosage, time to be given and practitioner's name.

Non-prescription (over-the-counter) Medications

1. A parent/guardian permission to administer medication consent form.          

2. Medication must come in it's original, manufacturer's non-expired container.

NOTE: For nonprescription drug products requested to be given beyond the recommended dose and non-FDA approved products (such as herbal products, dietary supplements), authorization in writing by a medical practitioner must be provided to the school.

Additional Information

Parent/guardian must deliver all medications directly to the school health office. Do not send medications to school with your student. 

All medications must be stored in the school health office. Students are not allowed to possess, use, carry, distribute, share, or sell prescription or over-the-counter medications. 

Students are allowed to self-carry and administer emergency medications, such as inhalers and epipens, if indicated in writing by their health care provider and parent/guardian. 

For safety reasons, the school district will not administer the first dose of any medications, with the exception of emergency medications. 

No CBD products are permitted for use at school. 

A new physician order must be submitted to the school prior to and with any change in medication, dose, time, or route of administration and/or at the start of a new school year. 

All medications must be picked up by a parent/guardian within fourteen (14) days after the end of the school year. Any medications not picked up will be discarded, with the exception of emergency medications.
